www.rallydev.com I've used a few years ago, so I'm not sure how much it's changed since that version. It's the only Agile management software I've used so I can't really compare it to it's competitors, but it beats post-its and white boards when you're dealing with larger teams or distributed teams.
As for adopting the Agile methodology the process really has to be done as an organization. The tools won't help unless the culture can conform. Rally may make it easier to visualize how the Agile process works since it does generate charts and a spreadsheet like interface.
