[Refresh Austin: 4570] Help with secure contact form & CSS problem

Looking for php, jsquery & css assistance to complete a secure form that I put on a test page (see below). I uploaded the sample test.php, token.php into their own directory and added the css into main css file, but a. for some reason styles aren't working. b. I don't know jquery or php - or how to get the form working. c. and wonder if anyone can tackle this for me? d. Is there a better solution that handles visitors who aren't using _javascript_-enabled browsers? e.
