David, the EPP extension registry template looks fine. The only thing I'll suggest changing is the "document status" field value. Please use "Other" instead of "Informational" to avoid confusion with Informational RFCs. Your note doesn't say anything about the status of requesting registration of the proprietary XML namespace and schema URIs in the IETF XML registry. Please encourage the submitters to register those values, too.
