How to recreate:
1. login as admin,
2. go to "Add new user" pane.
3. add a new user with the role "everyone"
Following error message will be displayed.
Error adding user to role. Violation of unique constraint SYS_CT_143: duplicate
value(s) for column(s) $$ in statement [insert into um_user_roles (id, user_id,
role_id) values (?, ?, ?)]
it is better to remove the role "everyone" from the role drop down since system
is adding this role to every new user by default.
