[Remember Mailing List] how to customize the join form

Hi all, 

 

I've followed the tutorial for the samplemember and would like to
customize the join form. When I create a samplemember_edit.cpt template
in the skin however, it only modifies the edit form (when the user has
already registered).

 

How can I customize the join form (form returned by the createMember
function),
