Steve, This Motorola Duplexer will work great with your repeater. You should be able to make the cables yourself. They are made with good quality Amphenol PL259 connectors. Don't use the radio shack ones. Use the ones with the white teflon center insulator.
You will also need some good coax. Use RG142B which is double shielded and silver plated. I have seen a manual on the internet about this duplexer and I will try to locate it. I have a few of these in service and I could measure the jumpers for you if I need to.
