Looks like it is the DB-411-B model I have.  

I've sent out a few copies of my drawing already 
with the DB-402-B file ID. If you're unable to 
modify the file ID (name) without hosing the 
file, email me direct again for a properly labeled 
DB-411-B.pdf file.
