Chris, I had the same need a few months ago. Sal had a copy and sent it to me. I do not have a means to copy it but if all else fails I can get it copied commercially. My copy is not perfect; it would be much better to copy an original if one can be found. The application notes are very useful but the "Operator's Guide" has a lot more information in it.
