Rick is on the right path: DIPLEXER

This is what is used to separate/combine two different frequency 
spectrum into/from one feedline/antenna

Simply wire it up like this:

144.800 >--VHF Port--
                      \
                       Diplexer ---> Dual-Band Antenna
                      /
438.100 >--UHF Port--
