*Purpose*

The Office of Minority Health has a need to acquire one Microsoft Access
Software Developer on a time and materials basis to provide enhancements and
reporting assistance for the MS Access “Closing the Gap” software
application.  The CTG application is used by approximately 50 sites
statewide.  *This engagement is anticipated to last until September, 2009.
*

*Tasks *

*For each application involved, they will provide the following tasks: *

   - Report to the designated Project Manager, Data Processing Manager or
   Bureau Chief assigned to each specific project
   - Provide MS Access software development and maintenance
   - Provide MS Access reports development and maintenance
   - Develop and/or modify project documentation as required
   - Work closely with IT software developers and customers to develop
   requirements specifications when required and to provide software that meets
   the needs of defined requirements & specifications.
   - Provide technical support as needed for users

*Submission of Resume*

   - A background check including fingerprinting will be completed on the
   consultant working on this project prior to the contractor commencing work
   - The selected candidate will comply with TIME IT submission requirements

   - The Department of Health will not contract with a vendor that
   restricts employment of its employees or contractors after conclusion or
   termination of a Department purchase order.  By responding to the purchase
   order, vendor specifically agrees to this provision.
   - It is the Vendor's responsibility to submit only resumes of staff
   members who meet US Dept. of Labor employment eligibility requirements, and
   eligibility requirements (Form I-9) of the Department of Homeland Security,
   U. S. Citizenship and Immigration Services.
   - Resumes must be received by close of business (5:00 PM Est.), Thursday,
   April 30th, 2009.
   - Selected candidate must comply with all project policies and
   procedures.
   - Reporting for emergency duty is required as part of your position.
   Emergencies may occur at any time and create varying degrees of damage,
   human suffering, injury, death, and property damage or destruction.  The
   Department of Health, along with many other local and state agencies, has
   responsibilities before, during and/or after emergencies.  As a DOH
   contractor, you are an important part of our emergency responsibilities.  In
   the event of an emergency, unless you are granted a temporary exemption from
   emergency duty, you may be required to work before, during and/or beyond
   your normal work hours or days to perform emergency duties including, but
   not limited to, responses to or threats involving any disaster or threat of
   disaster, man-made or natural.
