Can you try pulling the authenticator out of the login-config?  Also,  
try using:

<authenticator>
   <type>com.foo.MyAuthenticator</type>
   ...
</authenticator>

<login-config>
   ...
</login-config>

Also, you should be extending your authenticator from  
com.caucho.server.security.AbstractAuthenticator instead of  
implementing ServletAuthenticator directly (if you're not already  
doing that.)
