There was one additional instance of this fixed in 3.0.16. Can you upgrade
to the latest release, run fixreviewcounts
(https://www.reviewboard.org/docs/manual/1.7/admin/sites/management-commands/#resetting-review-request-counters),
and see if you can still cause this?
