If you submitted patch using post-review, the extended_path should
include version information (the fabulous '@@'). In your traceback it
appears to be missing.
In that case, for Dynamic Clear Case Views, it will try to apply the
patch on current version of the file, so it breaks when both:
a) file version changes (for example, review gets shipped)
b) the old file version disappears from cache

When using postreview the patches should include oids, and then the
ReviewBoard should output correct version and extended paths. You can
check if your patches contain the oids, by downloading those and
checking for lines starting/ending with four equal signs (====).
