I have managed to get remote hardware debugging working. I haven't seen your errors specifically, but I've found:
1) it wouldn't work if the Qsys design filename has upper-case letters in it. E.g. SOC_System.qsys wouldn't work, but soc_system.qsys would. 2) Some versions of Quartus didn't seem to work. I can't remember which, but it was around 14.1. I was also using Windows 8.1 at the time, which wasn't supported.
