The hacked mkinitrd script I use for dm-multipath root on RHEL4 basically includes enough of an environment in the initrd image to run multipath exactly as you would from a fully booted system. I've been using this for quite a while now and it has proved to be quite robust. I released this hacked script on my website about six months ago and I've had quite a few success reports since then. I'm not sure what a more appropriate method would be, but I'd be interested.
