What problem are you having with the form? I'll agree that the form
could use a little work to be more international friendly, however, that
form as it exist can easily be filled out by someone outside of the US.
Simply pick your country and, for the state, you can pick "Outside the
US/Canada". The phone number suggest a US format but will accept any
valid number.
I looked at the top of the list of states.
I looked at the bottom of the list of states.
Oh, there it is. How bizarre, right in the middle, under "O" for "Other."
