I installed the mpt-status RPM.  It provides output like the following:

# mpt-status
ioc0 vol_id 0 type IM, 2 phy, 135 GB, state OPTIMAL, flags ENABLED
ioc0 phy 1 scsi_id 4 FUJITSU  MBB2147RCSUN146G 0505, 136 GB, state ONLINE,

# mpt-status -s
log_id 0 OPTIMAL
phys_id 1 ONLINE
phys_id 0 ONLINE

Which is easily parsable with a cron job of your own writing. I got it from http://www.drugphish.ch/~ratz/mpt-status/RPMS/1.2.0_RC7/ and it has been rock solid for years now.

Things I had to do to make it work (you may not need to do the latter two):

 rpm --install mpt-status-1.2.0_RC7-3.i386.rpm
 echo "alias char-major-10-220 mptctl" >> /etc/modprobe.conf
 /sbin/modprobe mptctl
