flacattack works very well, but you have to configure it properly. In
EAC, under "Compression Options", you'll want to have something similar
to this set in the "Additional command line options" field:

"C:\Program Files\flacattack\flacattack.ini" %s "%a" "%t" "%g" "%y"
"%n" "%m" %o

Once you have EAC configured to use flacattack, you'll need to set up
flacattack. Just check all of the boxes under the "Command Line
Options" section of the FLAC tab, and make sure you save your config
file after you make changes, since it doesn't save it automatically.

It depends on how you want the albums displayed. The fact that a live
recording is split over many discs is completely arbitrary -- the live
performance wasn't created with the CD in mind, so just treat the whole
thing as a single show and number it accordingly. You're ripping the
music to FLAC, so you can always put the files back onto discs later in
any way you want. 

I usually just number the songs from whole from 1-n afer its been
ripped, and put a d1, d2, d3 in the filename after the fact in case I
want to re-create the discs from the FLAC files. Just number the files
d1-nn, d2-nn, d3-nn where nn is the number of the track from 1 to the
total number of tracks in the show. This will let you see where the
disc was split, but maintains a number scheme so that the show plays
from beginning to end.

You should always use replaygain, if you can spare the time it takes to
calculate. It'll make for a much better listening experience. So, yes,
check the box. If you have metaflac installed and are using flacattack,
the rest will be handled automatically.
