Well, it's a guess...but the odds of a bad rip on every single track on an undamaged CD are astronomically low.
Even on a damaged CD, not every track will have errors. But unfortunately this will mask any errors you do have, so it's remotely possible one could sneak through without you knowing it. This is why I can't use dBpowerAMP R12 - it relies exclusively on AccurateRip. However many of my CDs are produced in this country so they don't match what's in the AccurateRip database. About 25% of the discs I rip report exactly like you describe.
