Have you checked the album folder for multiple image files? SS checks for album art and thumbnails using a different order for each. See http://wiki.slimdevices.com/index.cgi?AlbumArtwork for more info.
If you've changed or added artwork, you might have to do a clear and rescan in order to pick up all the changes - especially the thumbnails.
