I keep my main library in Flac and then run a script to transcode the
whole library to an Mp3 mirror.  On my PC I usually use Winamp to
listen to music files for editing, testing, etc.  I find that about 1
in 100 of the Mp3 files that I'm creating will not play in Winamp. 
They load OK, I can read the metadata, but when I try to play them, I
get no audio output and the progress bar doesn't advance.  The odd
thing is that I _can_ play these same files in Windows Media Player,
foobar2000 and Quicktime.

Here's one of the 'bad' files, if anyone wants to look at it and maybe
tell me if there's anything obviously wrong in the file structure or
somesuch (about 5 MB):

To create these files I decode the Flacs to WAV, then encode using LAME
3.97 at -V2 -vbr-new, then use metamp3 to add additional id3v2.3 tags
that LAME can't do.
