Only "clear library and rescan everything" seems to trigger an album-art scan. Could it be as simple as that? Also, the scanner needs capitalization to match (non-intuitive if you're coming from the Windows world), so it needs to be -cover.jpg- *not* -Cover.jpg- or -cover.JPG-.
