Either flac.exe or lame.exe is crashing. Usually, this is caused by a
corrupt flac file. Some suggestions:
1) Save the file that causes crashes for future testing.
2) Re-rip the audio to flac and try again.
3) If after re-ripping, you are still getting crashes, try each program
individually, to determine which is crashing. Here are the two
commands, simplified:

lame.exe --preset standard --replaygain-accurate --quiet - "test.mp3" <
test.wav

If it is flac.exe that crashes, you might want to take this up with the
flac developer, and have the crashing file available for analysis.
Likewise, if the test.wav file is created successfully, and it is
lame.exe that crashes, contact the lame (b) developers. You may need to
set your PATH appropriately. Ask for help if you do not know how to do
this.
