You've done a rescan, right? Try 'look for new and changed music' first,
then 'clear library and rescan everything' if that doesn't work. You do
this from 'server settings'.

If you have, tell us about the image - it's size in bytes and
resolution and also the image format (jpeg, png, bmp etc).
