My solution depends on the album and its context.  I also use my files
in multiple devices and software, so I'm somewhat limited to what I can
do in terms of tagging (Squeezebox is way more flexible than my PMP).

So, for live albums, I tend to rip everything into one folder, I give
it all one album name, and my track numbers are preceded by the disc
number. I can't use the discnumber tag itself or my PMP gets confused) 
For albums that work well as all one unit (e.g. The Wall, Bitches Brew,
The White Album), I do a similar item.  

For albums that can be toss-ups in terms of splitting or not, I will
often use an m3u list to separate the discs giving me a quick option
for listening to a specific disc.  I supposed I could just keep them
separate and then queue them up one after the other as well, but my own
listening habit tends to favor the whole

For albums that I want to keep separate, I keep them separate including
the album title.  For example Melancholy and the Infinite Sadness is set
up as 2 separate albums with different titles.  

Last, I do have some albums and classical music where a disc has two
distinct works or albums.  The Beach Boys had released some past
"Brothers Years" discs as two albums per disc.  I ripped them to one
folder but differentiated the album title for the appropriate tracks. 


The beauty of all this is you can do what you want and as long as you
don't mess things up too badly or take advantage of comment fields, you
can always change your setup pretty easily.
