I just responded in another thread the possibility of the track number
formatting in the tags and the potential for inconsistent disc numbers
within one album:

I would take one album that is giving you problems and check that the
disc number tags are either not there at all (not necessarily needed
with 1 disc albums) or that they are all identical.  

Aside: in the past, some software on my computer would try to
automatically fill empty tags on my audio files.  Since most of my
music did not have disc number tags, the software would fill in some
(not all) mp3's with files.  It wasn't unusual for me to have an album
with all but one track set with DISCNUMBER = 1 (or other variations of
that tag: DISCNUM, DISCNUMBER, DISC NUMBER, etc.).  It was those odd
tracks that were now missing a Discnumber that ended up sorted out of
place.  Similarly, a few tracks might end up with 1/1 vs. just 1, and
other variations.  

I'm not sure what program did it as it was on my computer, so it was
either ITunes, WMP, or MediaMonkey.  My guess it was one of the first
two.  Regardless, I had to either delete all the DISCNUMBER tags where
not needed or set them to a specific value that was consistent for a
given disc.
