My preferred Windows tagging application is Mp3tag. In cases such as
this I usually load all the files from all discs into Mp3tag, then
make sure that the album title is the same across all, then I use
auto-number (CRTL-K) to apply a sequential track number across all
tracks. Also, all files are within a single directory (ie no
subdirectory for disc 1 / disc 2 etc).

It's fairly easy to miss minor differences in album titles such as
spacing, capitalisation or punctuation ... but I can't guess as to why
you have the problem that you are reporting. I've *never* used <disc
number> tags.
