This is usually a permissions and/or apache problem.

First of all check that apache is serving the rivendell pages:

http://localhost/rd-bin/addcart.html

If you get a 404 you haven't added the rd-bin.conf to apache conf.d.

If you get a page, check that /var/snd has the right permissions. I 
think I normally use 775 as the permissions but that does leave it 
pretty open so test with that and then lock it down until it breaks.
