Coming from a Windows guy that still uses floppies....

If Win2k/XP are asking to reformat the disk, then they may be damaged.  
I have not had good luck with keeping floppies for extended periods of time.

You *might* try this disk in several different machine (different floppy 
drives) in the hopes that one of them will read them.
