Are you by any chance supplying a custom command-line to the boot loader when 
you start the installation?  The Appliance setup utilizes the CentOS 
‘kickstart’ mechanism to provide its customization; be sure that you have a 
line like ‘ks=cdrom:/ks/rivnedell.cfg’ on the boot command-line.
