I'm not sure what you mean by "audio ports tab"; does the card show up in Audio resources under rdadmin?
Are you using a new motherboard? I've had better luck with the Appliance on older hardware than on new hardware. CentOS 6 often doesn't recognize critical components of new motherboards, in my experience.
