Open Source code has to be clean because everybody sees it.

My limited experience with reading Linux kernel code has been otherwise. The
first time I looked at it (a Mandrake distro from around 2 years ago) I was
surprised to find comments like, "this is broken," and "fix this later."

I think the real problem is that human beings are involved; whether they
work for open source projects or proprietary ones is immaterial.

This is true, and comments like this exist in most open source code. The big difference with Open-Source is there are literally thousands of eyes on that code and it will be fixed and posted very soon. In the case of proprietary, there are only 40 or so eyes on it (in most cases) and they are all just as stumped as the original author - and they have nowhere to turn for help without violating thier non-disclosure agreement and putting themselves at risk.
