You can use   EVENT[GUARD]/ACTION  UML syntax for transition labels.

Hello everybody,

how would you incorporate exceptions into Activity diagrams? Let's
assume, you should design workflow process having several Activities,
which could lead (under certain circumstances - exceptions) to
termination of the whole workflow process. Is there any special UML
modelling mechanism for this case (, besides Decision model
elements...)?
