I also ran into this using a LADSPA plugin on top, after I had already gotten the synth plugin into a playable state. I applied the LADSPA plugin, hit play, listened, played my keyboard a bit, then paused for some unspecified period. (All of which sounded fine.) When I came back, it was all juddery again, and hitting play didn't fix it. I had to strip off the LADSPA plugin, hit play, and then reapply the LADSPA plugin. (I'm a little sketchy the morning after.
