I'm not taking any particular view on things like directory structure
for this branch at the moment, but it did bother me to lose all the
history -- so I've re-committed with the history from the previous
rtmidi branch.  This is still a bit problematic since the latest trunk
code has at some point been merged in "offline" with no history
checkpoint, but it's better than nothing.

Haven't had a chance to try building it yet though!
