It's a garbage in, garbage out problem.  We're feeding LilyPond garbage, and 
it's spitting garbage out.  There are countless possible scenarios that can 
cause this, and they aren't necessarily the user's fault.

None of it is as robust as we'd like it to be, but it's as robust as we can 
make it.  All you can really do is piddle around until you find something that 
finally works, as was the case here.  Neither one of us will ever really know 
why what you did worked.  Run with it.
