Hrm ... this breaks backwards compatibility of the user interface and it
changes the current default behavior. I'd rather suggest to do the
following:

 - keep the --no-header|-n option
 - create a DTD header by default
 - use --header|-h to change that default
 - complain if both, -n and -h are used

This should preserve the old behavior, thus not breaking possibly
existing scripts and you may _explicitly_ specify whether to create or
not to create a header, which imho is a much nicer interface.
