Untested example::fromhost-ip, isequal, "xxx.xxx.xxx.xxx" :ommysql:host:db:user:pass
:fromhost-ip, isequal, "xx.xx.xx.xx" :ommysql:127.0.0.1:dbname:dbuser:dbpassword
I'm out of ideas (never used ommysql)... did you put it on the same line? (your two lines are hard-wrapped in the mail source, so this could be the problem).
