I've gotten as far as determining that the culprit is the following
chunk of code in Approval/Approve:
% if ($ShowHistory) { <& /Ticket/Elements/ShowHistory, Ticket =>
$approving, Collapsed => 0, ShowTitle => 0, ShowHeaders => 0,
ShowDisplayModes => 0, ShowTitleBarCommands => 0 &>
% }
If I remove this, everything displays correctly.
