Finally I was able to reinstall RT and start from scratch, which is clearly not an ideal solution, but your files then worked like a dream. Unfortunately I'm still unsure what the actual reason for this non-functionality was but am somewhat suspicious of having added a new status (pending) via RT_SiteConfig.pm. Perhaps this shouldn't have had any effect, but I'm not convinced as the approvals also appear to be behaving somewhat better now (see other thread...)
