I, like Stephen and yourself recently upgraded to 4.2.4 and noticed pasting had gone AWOL, but interesting not all the time. It's a bit quirky, so initially put it down to something I was doing.

Browsing through that folder hierarchy, I can see that both the clipboard/dialogs/paste.js and the pastefromword/filter/default.js exist. Permissions and ownership are root:bin and 0644, respectively. The upgrade didn't report any errors so not sure where to look for problems. I'm assuming the issue is isolated to a relatively low number of installs.
