Code looks good. Are you sure that card really requests IT?
Try to check the IRQ3 bus line with some digital scope.
Try IRQ5 if possible. Try a plain motherbord if you have some
special HW now.
I had similar problems a few months ago. Finally I figured out,
that the superintegrated, superintelligent, all-in-one single board
industrial PC did not listen IRQ bus lines of built in peripherals
even if they are disabled by setup.
The DMA controller was simply disconnected from the bus. :-(
