I'm not sure, exactly, what you mean by 'email web form' but when I type 'rails email' in my Firefox Google search bar I get a number of recommendations including 'rails email form'. You might try that. In general, I think you'll find that you get a lot more help here if you ask questions that can't be immediately answered by Google.
