Let's give it a week or so for comments about naming and in general things

The old instructions will still work fine, the stubetc file system will
simply be mounted on top of the builtin /etc. The minimum short-term work
is to install the default /etc so that it's easy to create customized
versions of the contents ... but I've just about had enough of makefiles
for 2016 already.

FYI I've switched one of the Node.js examples to cookfs:

https://github.com/rumpkernel/rumprun-packages/blob/master/nodejs/README.md#baked-in-filesystem

If you want to have a single image to distribute, the partitioning support that someone was working on might be a better option than loading all data into wired memory. Of course, the former is not yet available ;)

Anyway, I remembered that I wanted to add a warning to cookfs for large image sizes. It now warns if you cook >1MB.
