I have been using urxvt with the font DejaVuSansMono:pixelsize=14 for some years, under Scientific Linux 5 and 6 (equivalent to RHEL 5 and 6 or Centos 5 and 6). I have recently installed Scientific Linux 7 and rxvt-unicode.x86_64 0:9.20-2.el7 from the EPEL repository. In this environment, the underscore character does not display in urxvt, although it always did before. The underscore does display in other fonts, e.g., lucidatypewriter, and I suspect that the difference may be related to the fact that the underscore is actually below the bottom of the alphanumeric characters in DejaVuSansMono, and not in the other fonts. (On the other hand, the descender of the character y does descend in DejaVuSansMono and it displays.)

I think the problem may not be in urxvt as such, but in an interaction between urxvt and something in the SL7 environment. I have tried an experiment compiling urxvt from source under SL6 and running the same binary in both SL6 and SL7. In SL6, the binary displays the underscores and in SL7 it does not. I don't know what all the changes are between the two distributions that could affect urxvt.

Underscores do display with DejaVuSansMono in UXTerm under SL7.
