No mention of sqlite, so you have probably included sqlite directly in
apsw. Not elegant, but it works and ensures that you get the version you
want.

Yeah, 'fetch' means no dynamic linking.

Yes, that's probably the problem.
You could try something like
CFLAGS="-I/sqlite_directory/include -L/sqlite_directory/lib
-Wl,-rpath=/sqlite_directory/lib" setup.py build_ext
