The usual way is:
(*) for very small patches, just send a mail with the attached patch
explaining it to the relevant developer mailing-list
(*) for normal patches, open a bug report and attach the patch explaining it
(*) for big changes, do all of the above, but also make available a
repository of git/svn/whatever (the one most convenient to upstream
maintainers) with a nice branch with tidy patches
Of course, some upstreams might have more peculiar needs (don't start me
on this...), but on a general basis people are quite welcoming of
sensible contributions.
