I recommend doing what is called a "hard cache reset".  That means clearing 
your browser of all files (including, in particular, javascript) related to 
the notebook - different browsers have different keyboard shortcuts or 
preferences to delete the cache.  This is a little different from what we 
usually see in such cases, but it can't hurt and probably will solve the 
problem.
