This may actually be a bug in {{{echelon_form()}}}. Manual inspection of
one of the failing matrices shows that {{{A.echelon_form()}}} doesn't
actually return a matrix in HNF, but that the row spaces of
{{{A.echelon_form()}}} and {{{L.echelon_form()}}} are in fact equal.
