 I just found something.  Is this what you are describing?  I don't see it
 all the time, but I did manage to make it happen once, so should be able
 to trace it.

 1) Make a worksheet with lots of cells that evaluate to produce a jmol
 plot.

 2) Do either of the following:

     a) select "delete all output", then select "evaluate all"

     b) select "evaluate all" without first deleting the output

 3) What I see sometimes is that one of the applets does not load and then
 many of the cells do not evaluate to a jmol applet.  They can be recovered
 by manually clicking the evaluate button.
